What Is The Upper Back
If you want to understand your upper back pain, start with an anatomy lesson.
Pain in the upper and/or mid back is less common than lower back or neck pain. One 2015 Mayo Clinic review of studies suggests that about a third of people get lower back or neck pain , compared to less than one-fifth reporting upper back pain.
The upper back is the region below the cervical spine and above the low back . The upper back is called the thoracic spine, and it is the most stable part of the spine. The range of motion in the upper back is limited because of the spines attachments to the ribs .
Think of your spine as a tree trunk. It keeps you standing upright. It connects parts of your skeleton to each other. It carries the weight of your upper body.
Since thats a massive job, the spine itself shares some of the physical load with nearby muscles, some of which include:
- Trapezius: Near your shoulder blade, helps you stand straight and throw
- Latissimus dorsi: Lower on your back, helps with arm movement and breathing
- Rhomboids: Adjacent to trapezius, supports your shoulders and helps you pull
When you hurt your upper or mid back doing yard work or playing tennis, chances are high youve injured one of these muscles. The most common reason we see people with upper back pain is a simple musculoskeletal strain, says Reginald Knight, MD, Director of Bassett Spine Care Institute in Cooperstown, NY.

What Causes Shoulder Pain

Some particular diseases which also trigger the shoulder pain like cervical spine diseases at neck or conditions of heart, liver and even gallbladder.
Hot Cold And Electrotherapy

Heating pads, ice or cold packs, and electrotherapy are natural ways to get relief from neck and shoulder pain. A general rule of thumb suggests choosing different therapies based on how long youve been experiencing pain.
Use cold for recent neck or shoulder pain
During the first six weeks after a fresh injury, experts recommend applying ice to help reduce inflammation and pain. Ice packs, cold sprays, and/or cold gels are great options to relieve acute swelling.
Use heat for chronic aches and pains
Heat encourages blood flow and helps relax tight muscles. This type of therapy can be introduced six weeks after an injury, and also works best for lingering aches and pains. Make sure your heat therapy isnt too hot. Try:
- Heating pads
- Warm baths, whirlpools and saunas
- Warm baths with Epsom salts
Try electrotherapy for short and long-term neck and shoulder pain
TENS, or Transcutaneous Electrical Nerve Stimulation, is a drug-free alternative to pain relief that works for both short- and long-term pain. TENS units use mild, pain-free electrical currents to disrupt pain signals.
Make sure you dont place the TENS unit pads too close to your head or sides of your neck and spine. Please follow the manufacturers directions for the best and safest pad placement.

What To Do If Your Shoulder Hurts
If you do experience shoulder pain, it’s not always possible to figure out the cause. Sometimes problems in other parts of the body are actually the source of the pain, which then radiates to your shoulder. This can happen with certain neck problems, including arthritis and disc herniations, says Dr. Arun Ramappa, associate professor of orthopedic surgery at Harvard Medical School.
In general, if you are experiencing a lot of pain or know you injured yourself, it’s best to see a doctor right away to have your shoulder condition diagnosed.
But if you have general, mild shoulder pain, try adjusting your activities, taking nonsteroidal anti-inflammatory drugs, and performing mild stretches to see if the pain improves on its own. However, if the pain doesn’t go away after a few weeks, you should consult your doctor.
Lower Neck And Upper Back Stretch

- Drop your chin toward your chest.

When To See A Doctor
Immediate medical attention should be sought for unexplained pain in the left shoulder blade accompanied by shortness of breath or chest pain.
In many cases, a person can trace shoulder blade pain back to a seemingly harmless incident, such as a recent fall or an occasion when they lifted too much weight.
It may be less evident to a person who slept badly, but warning signs include sleeping on one side for too long or having recently switched mattresses.
People who are unable to explain a pain in their shoulder blade should see their doctor.
Women should be particularly wary of any pain in the left shoulder blade as it could be a sign of a heart attack, and they should seek out immediate attention from a doctor.
A person may also wish to see a doctor in the case of a strain as the doctor may be able to recommend additional treatment.
Symptoms that signal a need for immediate attention include:
- chest pain
Other means of prevention include:
- eating a balanced diet that is rich in nutrients
- getting moderate exercise

Calcific Tendonitis And Calcific Periarthritis
Sometimes pain, swelling and difficulty moving your shoulder is caused by calcium crystals that form insidea tendon. Tendons are the strong cords that attach muscles to bones.;;

If they form in tendons, this is called calcific tendonitis.;;
This can happen at various places around the body, including the shoulder.;;
Calcium crystals can then shed from a tendon and cause pain and swelling around a joint. This is called calcific periarthritis.;;
Steroid injections often work well to reduce swelling. Butsometimes the calcium deposits mayneed to be removed by keyhole surgery.;

Pain In One Side Of The Neck
Stretching may help to ease shoulder blade pain.
Isolated pain in the right or left side of the neck is most commonly the result of muscle strain or injury.
The following activities could contribute to pain in one side of the neck:
- Bad posture. Poor posture can strain the muscles in and around the neck and may also lead to an uneven spreading of weight, which could cause pain on one particular side.
- Poor sleeping position. Sleeping in an awkward position can cause neck pain, and this is more likely for people who sleep on their stomach. Using too many pillows or an unsupportive mattress can also lead to problems.
- Whiplash. Pain from neck sprains are the result of ligament or muscle damage. While whiplash is most commonly associated with being in a vehicle accident, other common causes include doing vigorous sporting activity and riding a rollercoaster.

How Can You Get An Upper Back Pain Diagnosis
How to Fix Shoulder Blade Muscle Pain, Part 2When you go to a doctor for your upper or mid back pain, youll start with a conversation. Expect the doctor to ask you about any events that may be directly responsible: A fall, a car accident, or any changes or increases to your fitness routine.
You might be asked to rank your pain on a scale of 0-10. You should also be prepared to talk about how your pain does, or does not, interfere with the activities of your daily life. Can you cook dinner, get dressed, take a shower?
Then comes the physical exam: the poking and prodding part of your visit. The doctor wants to see how your upper back pain affects your movement. Expect to sit, stand, reach, and bend in the exam room. After that, youre likely to get an X ray.
If symptoms persist or you have red flags such as a progressive weakness or bowel or bladder control problems, you may get more advanced testing, Dr. Knight says. He explains that MRI would be the likely choice.
