Traditional advice for treating sciatica recommended complete bed rest. However, recent Australian studies have concluded that lying in bed may offer only minimal improvement, or may even exacerbate the condition.Unless specifically advised by your doctor, remaining active may be the preferred option. Gentle exercises can help. Swimming is particularly useful, as it is not a weight bearing exercise so flexibility and strength can be improved without unduly stressing your back. Herniated spinal discs usually heal themselves with time.

Treatment
Exercise #: The Hip Hinge
Do 3 sets of 10 to 12 reps
How to do it: Stand with your feet hip-width apart, knees slightly bent, and hands on hips. Brace your coreimagine youre about to get punched in the stomach and tighten those abdominal muscles.
Without changing the position of your knees, bend at your hips and lower your torso until its almost parallel to the floor, or as far as you can comfortably go without rounding your lower back.
Pause, then gently pull / lift your torso back to the starting position. Be sure to squeeze your glutes and push your hips forward to lift your torso back to the original position. This ensures youre engaging your hip muscles instead of relying on your lower back.
Thats one rep. Do three sets of 10 to 12 reps, resting for 30 to 60 seconds between sets.

Sleeping On The Side: Second
The second-best sleeping position for sciatica patients is sleeping on the side. It is believed that sleeping on the side can provide the necessary relief from the pain, as well as that it can reduce the pressure on the lower back.
Because youre not sleeping directly on your back, there is no pressure on the muscles, disc, and the sciatic nerve. However, for this position to be beneficial, it is important to keep the spine aligned, the hips straight and of course, the knees curled up a little bit towards the chest.
Note: To achieve this sleeping position, make sure to lie on the side where the pain is not located so youll be sleeping on the painless side. Furthermore, you will prepare a pillow for the arm support and rest, an extra pillow for the neck are and of course, pillows for between the knees.
Then, you will push the knees a little bit towards the chest, keep them bent at a 90-degree angle. Then, place the pillow between the knees to decrease the pressure on the sciatic nerve.
For more support you can place a pillow under the waist area this way you can ensure the body stays in that position the whole night, without putting extra pressure on the lower back.

Non-steroidal anti-inflammatory medications can lower the inflammation in the nerves which are usually the cause of the flare-ups.

Exercise #: Knee To Chest Stretch
Do 3 sets of 10- to 30-second hold per side
How to do it: Lie on a mat with your legs straight or knees bent and feet flat on the floor.
Raise your right knee toward your chest, then gently pull the back of your thigh to draw the knee even loser to your chest. Hold the position for 10 to 30 seconds. Release, switch to other leg, and repeat for a total of three sets per side.
Make it easier: If you have trouble getting onto the floor, you can perform this lying in bed or seated in a chair.

When coming into our office isnt possible, or you need sciatica pain relief immediately, make use of these sciatica exercises to do at home. The only equipment these at-home sciatica exercises require is a foam roller and a lacrosse ball.
- Glute Foam Roller: You can do this at home stretch with a simple foam roller. As youll see in the video above, Ashley is using the foam roller to roll over her left glute with her left leg extended. You want to focus on one glute muscle at a time so you can use the force of your full body weight to press down onto the foam roller. Spend 30 seconds to a minute on one side and then switch to your other side.
- Piriformis Stretch: As you can see in the video, Jane pulls her left knee toward her opposite shoulder in order to release tension in the piriformis muscle. If the piriformis muscle is tight, it can put pressure on the sciatic nerve resulting in sciatica pain. Spend 15-30 seconds per side on this stretch but be sure to avoid the bad stretch posture that Jane demonstrates. Its important to keep the hips down on the ground and pull the knee to stretch the piriformis and avoid stretching the back.

How Do I Know If I Have Sciatica Or Piriformis Syndrome
Both sciatica and piriformis syndrome cause sciatic nerve pain and have similar symptoms, the causes of the conditions are different.
Piriformis syndrome is caused by anatomic changes in the piriformis muscle. The muscle is located across the femur and outer section of the hips and rotates the hip. The syndrome causes buttock pain or spasms and can irritate and compress the sciatic nerve, leading to sciatic pain. Unlike sciatica, the pain begins in the buttocks, not the lower back.

Many people think that the worse the pain, the more likely something bad is going on. However, this isnt true for sciatica. The body can reabsorb the disc material that is causing symptoms, even for those with severe pain. So, treatment focuses on controlling pain and keeping people as active as possible. If the pain is excruciating, lying down for short periods can help, but prolonged bed rest does not. So, once the pain diminishes, I tell patients to get up and start walking short distances. Since sitting increases pressure on the discs in the lower back, I recommend avoiding prolonged sitting or driving. Many people try treatments like physical therapy, massage, acupuncture, and chiropractic manipulation, but evidence suggests that while these approaches may help typical low back pain, they are less helpful for sciatica. Over-the-counter pain medicines like ibuprofen and naproxen can help. When they dont, I may recommend short-term use of stronger, prescription pain medicines.
Patients often ask about spinal injections where steroid medicine is injected into the affected area. It is worth considering for those with uncontrolled pain or for those with persistent, bothersome symptoms who want to avoid surgery. Injections can provide short-term relief. Like any procedure, it has uncommon risks including more pain, and it doesnt seem to decrease the need for future surgery.

What Is The Role Of Surgery In Sciatica
Surgical intervention for sciatica focuses on removal of disc herniation and eventually part of the disc or on foraminal stenosis, with the purpose of eliminating the suspected cause of the sciatica. Treatment is aimed at easing the leg pain and corresponding symptoms and not at reducing the back pain. Consensus is that a cauda equina syndrome is an absolute indication for immediate surgery.
