Where Carpal Tunnel Pain
The median nerve in the wrist is responsible for the pain, tingling, and numbness that can happen when you have carpal tunnel syndrome. Occasionally, you will have shock-like sensations that radiate to the thumb and index, middle, and ring fingers. If you have pain or tingling, it may travel up the forearm toward the shoulder.

Incision Pain And Pillar Pain
Pain in the area of the incision is especially common after traditional open surgery, rather than endoscopic carpal tunnel release. There two types of pain that occur in the palm of the hand after carpal tunnel surgery: incisional pain and pillar pain.
Incisional pain occurs directly at the site of the incision. Pain directly at the incision is typically only present for days or weeks after the surgery.;Protecting the incision can help alleviate pain, and it’s important to avoid lifting or gripping for several weeks after carpal tunnel surgery.
Pillar pain is the pain experienced to the sides of the incision in the thicker parts of the palm, called the thenar and hypothenar eminence. Pain in these regions is where the attachments of the transverse ligament to the carpal bones are located.
In addition, the muscles of the palm of the hand are located here. Pillar pain is the more common and troublesome complication of carpal tunnel surgery and may take several months to resolve.
Treatments for pillar pain may include rest, massage, and hand therapy.;Additional surgery is generally ineffective for treatment of pillar pain.
Who Is At Risk For Carpal Tunnel Syndrome
People at risk for carpal tunnel syndrome are those who do activities or jobs that involve repetitive finger use. Motions that can place people at risk of developing carpal tunnel syndrome include:
- High-force .
- Extreme wrist motions.
- Vibration.
Many other factors can also contribute to the development of carpal tunnel syndrome. These factors can include:
- Heredity .
- Pregnancy.
- Amyloid deposits .
Carpal tunnel syndrome is also more common in women than in men.

How Well Does Surgery Work
Most people who have surgery for carpal tunnel syndrome have less or no pain and numbness in their hand and better hand function after surgery.footnote 1 It’s not clear how long you should try nonsurgical treatment before considering surgery. But after people who have severe carpal tunnel syndrome have surgery, they typically have fewer symptoms and better hand function than people who continue using wrist splints or a combination of medicine and hand therapy.footnote 2
Both types of surgeryopen and endoscopicwork equally well to improve symptoms.footnote 3 Talk to your doctor about which surgery might be best for you.
Major problems from surgery can happen, but they are rare. About 1 person out of 100 has a major problem after surgery, such as nerve damage, while 99 out of 100 people don’t.footnote 3
Steroid Injections For Carpal Tunnel Syndrome
Your GP may offer you a steroid injection into your carpal tunnel to help relieve your pain. You may be able to have this at your GP surgery or they may need to refer you to a specialist for the treatment.
Steroid injections work well for many people but it can take a few weeks to get the full effects. They dont work for everyone. They can also wear off after a while and your symptoms may come back. If your symptoms do return, you may be able to have a repeat injection but its not always effective.

Diagnosis Of Carpal Tunnel Syndrome
Your GP will ask you about your symptoms and your medical history. Theyll want to know when you get symptoms, what triggers them, and how theyre affecting your daily life. Theyll carefully examine both your hands, looking for signs such as swelling or muscle wasting. Theyll also check the feelings in your fingers and thumb. They may carry out some brief tests to check your symptoms. These may include:
- asking you to bend your wrist with your palm towards your forearm
- tapping or pressing over your median nerve on the inside of your wrist
- pressing on the ligament over your carpal tunnel in your wrist
Your GP may be able to diagnose carpal tunnel syndrome just by examining you and asking about your symptoms. But sometimes they may ask you to have some further tests. These may include the following.
- Nerve conduction test this can show how bad any damage to your median nerve is.
- Ultrasound this allows your doctor to look inside your wrist, and can help to show if theres any underlying cause.
Symptoms Of Carpal Tunnel Syndrome
Carpal tunnel symptoms tend to affect your thumb and fingers . Symptoms include:
- numbness
- a tingling feeling or pins and needles
- weakness and finding it difficult to grip
- pain or a burning feeling
You can have carpal tunnel symptoms in both hands or just one. It can start to affect your whole hand and may also spread up your arm. Your symptoms may be mild at first, but they may gradually get worse. You can get carpal tunnel symptoms at any time but theyre often worse at night and may wake you up. Certain activities may also trigger your symptoms in the day.
Your carpal tunnel syndrome symptoms may feel better if you shake your wrist or change its position.
If you have any of these symptoms, contact your GP for advice.

How Does Surgery Fix Carpal Tunnel Syndrome
Surgery reduces the pressure on the median nerve in the wrist. The doctor cuts a ligament to relieve the pressure on the nerve.
The surgery is called carpal tunnel release . It can be done in one of two ways:
- Open surgery : Your doctor makes a small cut in the palm of your hand.
- Endoscopic surgery : Your doctor makes one small incision in the wrist, or one small incision in the wrist and one in the palm. He or she puts a thin tube with a camera attached into the incision. Surgical tools are put in along with the scope.
You may decide to have surgery if you’ve had very bad symptoms for a long time and other treatments haven’t helped.
Sometimes surgery is needed to prevent lasting nerve damage. You will likely have a nerve test to see if you need surgery.
How Often Is Hand Pain Caused By Carpal Tunnel Syndrome
While carpal tunnel syndrome is a common condition, it has a different set of symptoms from many other sources of hand pain. There are actually several similar conditions that cause hand pain. These include:
- De Quervains tendinosis: A condition where swelling affects the wrist and base of the thumb. In this condition, you will feel pain when you make a fist and simulate shaking someones hand.
- Trigger finger: This condition causes soreness at the base of the finger or thumb. Trigger finger also causes pain, locking and stiffness when bending the fingers and thumb.
- Arthritis: This is a general term for many conditions that cause stiffness and swelling in your joints. Arthritis can impact many joints in your body and ranges from causing small amounts of discomfort to breaking down the joint over time .

About Carpal Tunnel Surgery
Many carpal tunnel surgeries are performed each year to help improve day-to-day activities. Endoscopic carpal tunnel surgery, performed by our leading surgeon Dr. John Michael Quinn, will help alleviate pressure and discomfort in the wrists. Advances in technology are making this surgery more effective and less invasive.
An endoscopic procedure uses a miniscule camera, allowing Dr. Quinn to view the internal structure of the wrist without having to open it up with a large incision. This procedure will help correct the carpal tunnel quickly while also minimizing downtime.
Test : Provocative Test For Carpal Tunnel Syndrome
The first test for carpal tunnel syndrome can be performed on yourself. Note that its best to have a partner help you. Doing so increases the accuracy of the results.
These are manual tests and are divided into 3 separate exams:Phalen, Tinel, andDurkan exams.Usually if only one of these exams are positive, then you likely have carpal tunnel syndrome. If more than one is positive, its nearly certain you have carpal tunnel syndrome.
These manual exams are described below.

How Is Carpal Tunnel Syndrome Diagnosed
Your doctor will probably ask you about your symptoms. He or she may examine you and ask you how you use your hands. Your doctor may also do these tests:
- Your doctor may tap the inside of your wrist. You may feel pain or a sensation that feels like an electric;shock.
- Your doctor may ask you to bend your wrist down for 1 minute to see if this causes symptoms.
Your doctor may order a nerve conduction test or an electromyography test. They use these tests to see whether the nerves and muscles in your arm and hand show the typical effects of carpal tunnel syndrome.
Can Carpal Tunnel Syndrome Go Away On Its Own
Are you hoping that carpal tunnel syndrome will vanish away on its own? According to some experts, if carpal tunnel syndrome is mild and is caught early, it can go away on its own provided that you are engaging in strict rest. However, if your carpal tunnel is not treated properly, you can have the aftereffects of permanent nerve and muscle damage. Carpal tunnel treatment is absolutely a vital decision to make if you are experiencing the effects of carpal tunnel.;

The Pain Of Carpal Tunnel Syndrome
The carpal tunnel is a passageway that runs from your wrist to your hand. It contains nerves and tendons. Carpal tunnel syndrome develops when theres prolonged pressure on the median nerve inside your carpal tunnel.
The median nerve controls feeling and movement in all your fingers except your pinky. When it gets pinched, a variety of symptoms can develop, including:;
- Pain
- Loss of function
- Reduced grip strength
A naturally narrow carpal tunnel or swollen tunnel from repeated hand motions can put pressure on the median nerve.
Lifestyle modifications can make a big difference to mitigate carpal tunnel pain. Take frequent breaks if your work requires repetitive wrist or hand motions. Ask Dr. Downing about exercises or stretches that may help condition your wrists.
Wearing a wrist splint can keep your wrist straight and minimize pressure on your median nerve. Some people find that wearing a splint at night gives their wrist nerves a rest and helps eliminate numbness and tingling.
To manage pain, anti-inflammatory medications or corticosteroid shots can be effective. With a comprehensive exam and discussion of your symptoms, Dr. Downing or Dr. Brereton can help you decide which treatment is best for your carpal tunnel pain.

How To Stop Carpal Tunnel Pain Without Surgery

Answer by Doctors from First Hand Medical:
Carpal Tunnel Pain and Numbness can be excruciating causing anxiety, worry and sleepless nights. It is so difficult to function when you are not getting enough sleep at night and your hands are painful, slow and clumsy during the day.
In fact, Carpal Tunnel often interrupts the most important restorative sleep known as REM sleep. When we go into REM sleep, our bodies go into a state of semi paralysis, so we dont act out our dreams during this deep stage of sleep. It is when a person with Carpal Tunnel goes into deep REM Sleep that their hands wake them up. CTS prevents sufferers from getting this most important stage of sleep.
After several days going with restricted REM Sleep, people get irritable and find it hard to concentrate.
As a side note, Intelligence agencies report that sleep deprivation over several nights in a row is the most effective type of torture to inflict on captives when trying to get important information. They have found this type of mental torture is much more effective than inflicting physical pain or injury.
In the case of Carpal Tunnel, when you combine this type of mental torture associated with REM Sleep Deprivation with intense hand pain, annoying numbness and the inability to function at your normal pace at work the anxiety can be more than most people can tolerate.

Modifying Your Hand/wrist Activities
Pressure on the nerve within the carpal tunnel is increased with forceful gripping tasks. It would make sense to take rest breaks and do stretches when you need to use the hands for heavy gripping. For example, when I am in heavy traffic and white knuckle gripping the steering wheel, my hands will go numb. A lighter grip on the wheel will help this calm down. Using vibrating power tools in the yard, such as a hedge trimmer or edger, can also flare up CTS symptoms. Gardening has also been shown in studies to contribute to CTS symptoms. Motorcycle riders often get CTS symptoms because they are forcibly gripping the handlebars and getting hand vibration from the ride. Common sense tells us to be careful with these activities and adjust how the hand is used when possible.

What If These Treatments Dont Help
In some cases, surgery is needed to make the symptoms go away completely. The surgery involves cutting the;ligament;that may be pressing on your median nerve. Youll usually get back the normal use of your wrist and hand within a few weeks to a few months after surgery.
Doing the hand, wrist, and finger exercises that your doctor tells you to do after surgery is very important. Without exercise, your wrist may get stiff, and you may lose some use of your hand.

Examination Of The Wrist
Your doctor or health professional will ask you to describe your symptoms. They will take a look at your hand and wrist to assess how bad the condition is. If the wrist is swollen due to arthritis or tendon swelling, this could be the cause of your symptoms.
If youve had the condition for some time, there may be signs of muscle wasting at the base of the thumb. If the problem is severe, your thumb, index and middle fingers may be insensitive or numb to either a gentle touch or a pin prick.
Your doctor may tap over the median nerve on the palm side of your wrist, this is known as Tinels test. Or they may ask you to bend your palm towards your forearm for up to a minute, which is known as Phalens test.
These tests can help to diagnose carpal tunnel syndrome, but they arent reliable, so you may also have one of the tests described below.
Myth: Only People Who Work In A Factory Or Type All Day Get Carpal Tunnel Syndrome
Work-related conditions that involve high levels of hand-arm vibration or hand force, prolonged work with a flexed or extended wrist, or high repetitiveness are associated with a higher risk for carpal tunnel syndrome, according to one systemic review of studies.1
See Causes and Risk Factors for Carpal Tunnel Syndrome
However, there is no clinical evidence that work-related factors can cause carpal tunnel syndrome. Experts believe that work-related stress may be the final straw for individuals who have other non-behavioral risk factors, such as female sex, advanced age, or a history of diabetes.

Women At Grater Risk Of Carpal Tunnel
There are some case studies done showing that women could be at a greater risk of 2 Corporal tunnel syndrome.
due to hormone imbalanceshormones play a big role in mens and womens biology and how the body can heal and rebuild itself.
This is why your diet can be very important to hormone production and why a healthy diet can actually help carpal tunnel syndrome. By having a healthy diet, your hormones will be better balanced, and with better balance hormones, your body will be able to repair better.
So the next time you eat that Indulgence, you may want to Second think about how this food could affect your hormones, which can affect your bodys ability to repair and leave down to Corporal tunnel syndrome?
Women tend to be at higher risk of hormone problems. I believe thats because they are the being that creates life the womans body does a lot more on a hormone level than a mans body. A woman has to create hormones for 2 when in pregnancy. this can be very taxing on a womans body, making a woman more susceptible to hormone issues and possibly carpal tunnel syndrome.
How Is It Managed
The initial line of management includes rest to the wrist, identification of the stress factors and avoiding it. Weight reduction, control of diabetes and rheumatoid arthritis, successful completion of pregnancy will all improve the condition. Wrist splints can be used to mechanically prohibit any unwitting wrist movement. Several medications in the form of tablets are given to alleviate pain. It is not uncommon to find relief with such maneuvers. Local injection of steroids to the affected wrist can be given to provide relief which is usually for a short term of few months. Surgery is indicated if the pain is very severe not relenting to medications or if weakness of the fingers is appearing. It is essential to get operated at the earliest if weakness is appearing as if muscle wasting occurs it is irreversible. The surgery known as carpal tunnel release is done as a day care procedure under local anesthesia.
